Hotel Reservations for the AHS 69th Annual Forum and Technology Display
Please make all hotel reservations at official Forum 69 hotels! This helps to defray the costs of the Annual Forum.
To receive a discounted Forum 69 room rate, you must confirm guest room reservations with the official hotel before April 25, 2013 (deadline extended). After midnight on April 24, Forum 69 room reservations will be confirmed on a space-available basis at the prevailing room rate. A deposit equal to one night's room and tax is required and will be charged by April 3, 2013 (or at time of reservation if after this date) to the credit card used to guarantee the reservation. All deposits for individual room reservations are fully refundable if a room is cancelled 72 hours prior to arrival.

All hotel reservations for the Hyatt Regency Phoenix, the official Forum 69 Headquarters hotel, must be made online on the AHS website. Making a hotel room reservation on this website is secure and guarantees your reservation at the Hyatt. It also assists AHS with achieving its contractual guest room pickup requirement.
If you are eligible to receive a government per diem rate please enter SOCGOV0513 in the access code field. A government photo I.D. is required upon check-in to verify eligibility. Individuals will be limited to five guestroom reservations under one name or credit card.
